There is a one time enrollment fee of $50
Monthly membership fee is as follows:
- Ages 0-18 $30 per month ($25 if a legal guardian is also our patient)
- Ages 19-44 $60 per month
- Ages 45-64 $75 per month
- Ages 65+ $85 per month
